Finasteride's Half-Life vs. Its Duration of Action: The Confusing Disconnect

Finasteride clears your blood in roughly 30 hours. It suppresses DHT for considerably longer — because the drug binds to an enzyme, and that binding outlasts the drug's plasma presence.

A drug's half-life is widely quoted and widely misunderstood in what it implies for clinical use. For finasteride, the disconnect between the half-life (short) and duration of action (longer) has real practical consequences that are worth understanding.

The short half-life

Finasteride's plasma half-life is approximately five to six hours. After a single dose, peak concentration is reached at one to two hours, then declines. After approximately five half-lives — 25 to 30 hours — the drug is essentially cleared from circulation.

If pharmacological effect were directly tied to plasma concentration, finasteride would need multiple daily doses to maintain DHT suppression around the clock. But this is not how enzyme inhibitors work.

The longer duration of enzyme inhibition

Finasteride is a competitive inhibitor of 5-alpha-reductase type 2. It binds to the enzyme active site and blocks it. The drug-enzyme complex dissociates slowly — more slowly than the rate at which the drug is cleared from plasma. As a result, even as finasteride concentration in blood falls, the enzyme remains inhibited for an extended period beyond what plasma levels would predict.

Studies measuring DHT at intervals after finasteride dosing confirm this: DHT suppression from a single 1mg dose persists for approximately 24 hours. Once-daily dosing maintains steady-state suppression throughout the day with modest fluctuation around dosing time — not the dramatic daily cycle that the six-hour half-life would suggest if you were tracking plasma levels alone.

What this means for missed doses

A single missed dose does not cause DHT to return to baseline in 24 hours. The enzyme remains partially inhibited from the previous day's dose; DHT rises gradually but stays below baseline until the next dose restores the usual suppressed level. Clinically negligible for one day.

Extended gaps — a week or more — are different. The enzyme inhibition eventually dissipates, DHT rises toward baseline, and the clinical effect erodes. But occasional single misses on an established regimen are not a treatment concern.

The practical rules

  • Missed one dose: take next dose as scheduled. No double-dosing needed.
  • Missed two or three doses: resume daily dosing normally.
  • Stopped for a week or more: steady-state suppression re-establishes in about two weeks of daily dosing on resumption.
  • Dose timing: consistency matters, not the exact hour. A few hours' variation is clinically irrelevant.

The dutasteride contrast

Dutasteride's half-life is approximately five weeks. DHT suppression from dutasteride persists for months after the last dose.
